Could be real. I get cluster headaches. They’re called that because they occur in clusters. Mine happen every year around early spring, and usually last less than an hour. It’s a damn miserable hour though.
This year they came on once or twice a day for two months. Usually I only suffer for roughly a month. The amount of people I had to explain why I needed to not talk and hold my head in a certain position, who then told me I was dehydrated and should have some water. Well I wanted to kill them. I know my body better than anyone. My piss was nearly clear, I was plenty hydrated.
Your mom probably knows her body and knows that the water will not help.
Fun fact, some people get cluster headaches year round and they can last for over a day. They are truly miserable. It’s like all the muscles in your temple and behind your eye tense to max and won’t let go. I’ve had black eyes from them. They literally squeeze the tears out of your eyeball.
I can’t imagine getting them worse than I do. They have a fun nickname too- suicide headaches. When they’re happening I have a recurring fantasy of drilling a hole in my skull to relieve the pressure. But it’s not actually pressure, just kinda feels like it.
Of course it can be real. But you want to remove all the easily manageable factors, like dehydration. We had this with my kids a few times:
My head hurts
Do you remember drinking water or other stuff?
Will the water help with my headache?
Could help, could not, but you can try
It was kinda hard to explain that this is not a game mechanic where you have a clear countermeasure to a negative status, but they more or less understand it now
Still not convinced. My ex also had them and I was sympathetic for many years, doing my best to help her through them.
But it was eventually too much. She identified a few things as likely triggers. I realize that’s not 100% but every time she had another episode, she exposed herself to a trigger. When she successfully avoided triggers, she had them much less frequently, even going years between. I eventually lost my sympathy: if you have good evidence of triggers yet don’t avoid them, then you FaFo
I apologize, that I didn’t mean to doubt your experience - sometimes there is something you can do, and if you don’t follow through with it, the rest of us will be much less patient. However I also realize there’s never a perfect way to prevent them and not everyone has identifiable triggers.
I had allergies bad as a child but they mostly went away in my mid 20s. Certain animals bother me but usually not plants. No food or medicine allergies.

If your mom has chronic headaches, chances are that she’s already tried and it doesn’t work.
As a migraine sufferer, I’ve never heard of more useless advice than ‘drink water’.

The only trigger I can identify is spring weather in a small city in a place that’s mostly temperate rainforest.
I apologize, that I didn’t mean to doubt your experience - sometimes there is something you can do, and if you don’t follow through with it, the rest of us will be much less patient. However I also realize there’s never a perfect way to prevent them and not everyone has identifiable triggers.
