    It always takes deaths or severe injuries before regulators realize the need to codify safety regulations. Safety rules are written is blood, as they say. Almost anything you think of that might be common sense, like “don’t stick your hand near a giant saw blade”, or “don’t enter a garbage compactor that’s in operation”, were put in because something like that happened at one point or another.
