This reminded me of the USS Missouri. In 1950 the battleship USS Missouri ran aground. An investigation found that the crew was so afraid of their Captain that nobody dared speak to him which caused the crew to fail to inform him when the ship was heading off course. The Navy reprimanded the Captain for creating a hostile environment, effectively ensuring he would never receive another promotion.
